Levels of Care

Once a decision to seek hospice care is made, we work with you and your family to help you stay at home – in your house, assisted living or skilled nursing facility – wherever you call home.

Routine Care at Home

Hospice team members visit the patient’s home/residence as needed to offer services.

Inpatient Respite Care

Provides temporary relief to the patient’s primary caregiver.

General Inpatient Care

For pain or other acute symptom management that cannot feasibly be provided in the home.

Continuous Home Care

Between 8 and 24 hours of care provided in the home to manage uncontrolled pain or other acute medical symptoms.

Our services include a nurse on call 24/7, pain and symptom management, location of resources, medical supply and equipment arrangements and a variety of offerings such as massage, reiki, reflexology and pet, music and art therapy. We work together with families and caregivers to offer the level of care required in a private way that is tailored to each individual’s needs.

Patients can expect: physical care, support of spiritual needs, emotional support, measures to ensure comfort, ongoing bereavement support for families and caregivers for up to 13 months past the point of patient death.
